Work Schedule:

Monday thru Friday, 8 to 5

 

  • Home Health Acute Care Patient RN Training provided.

Responsibilities

 

Provides skilled services to patients at home and manages the patient care services pertaining to the curative, rehabilitative, and/or preventive aspects of nursing pursuant to the plan of treatment when nursing is involved in a patient’s care.

Work Performed:

  • Makes the initial evaluation visit, when nursing is involved in the plan care and at other times, as directed by the Director of Professional Services;
  • Regularly re-evaluates the patient's nursing needs;
  • Initiates the nursing plan of care with appropriate preventive and rehabilitative nursing procedures and necessary revisions;
  • Furnishes those services requiring substantial and specialized nursing skill;
  • Prepares clinical and progress notes the day of service;
  • Coordinates services;
  • Informs the physician and other personnel of changes in the patient’s condition and needs
  • Counsels the patient and family in meeting nursing and related needs;
  • Participates in in-service programs;
  • Supervises and teaches other nursing personnel;
  • May delegate aspects of patient care to unlicensed individuals
  • Teaches and trains patients and caregivers nursing care
  • Provides nail care to patients
  • Performs medication management setup
  • Any other duty within the scope of nursing state practice regulations.

Qualifications/Skillsets

  •  Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
  • Self-directed and able to work with minimal supervision.
  • Management experience not required.  Responsible for supervising hospice aides.
  • Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching.  Visual acuity and hearing to perform required nursing skills.
