    The Full-Time Home Health Clinical Orders Liaison/Licensed Practical Nurse will be responsible for working with a team of office professionals assisting the Signature team of clinical professionals out in the field with orders, medical records, supplies, etc. The Clinical Orders Liaison/Licensed Practical Nurse will also work as a back-up for the Intake Scheduler as needed. This position is a full time office position working Monday through Friday and regular office hours.


    • Request orders, clarify orders, document and process clinical orders via phone and fax; track requested orders and follow up with MD offices; notify clinicians of process
    • Follow up with clinicians to get clear, legal orders
    • Communicate with scheduling to ensure appropriate room is made on clinician’s schedules
    • Medical records auditing and filing
    • Maintain Supply room for clinical professionals
    • Order patient specific supplies, manage medical supplies to keep in line with budget
    • Receive and track lab results, fax results to MD and send to clinical manager and medical records
    • Assist with Social Workers schedules, request evals, process and schedule orders
    • General contact point for questions from clinicians, intake and scheduling. Trouble shooting and research services.


    • Must possess a nursing degree from an accredited college or university.
    • Must possess a current, unencumbered, active license to practice as an LPN in this state.
    • Current CPR Certification.
    • Minimum 1 year recent experience in a clinical setting.
    • Must have a valid driver’s license and current automobile insurance.
    • Must possess the ability to work independently without supervision; must be a self-starter and use good judgment in difficult situations.
    • Must possess the ability to work in an ever-changing environment, remaining focused and adapt with ease.
    • Knowledge and experience interacting with residents of this nature.
    • Possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages.
    • Willing to be on call and place client’s needs first.
