    Full-Time Home Health Intake Coordinator

    The Home Health Intake Coordinator is responsible for managing all aspects of the patient/client intake process including assisting the members of the intake/admission team, establishing and maintaining positive relationships with customers and referral sources and responding to customer requests and concerns. This position would work in within patient admitting process for home bound patients and telehealth patients. Would like qualified candidate to have medical admitting experience, prefer telehealth experience, and would prefer Licensed Practical Nurse but not required for position. 

    Responsibilities

    • Ensure compliance of physician orders and pre-authorizations.
    • Coordinate patient/client visits, matching patient needs with staff talents and knowledge.
    • Maintain and upkeep scheduling records, patient charts and logbooks, and accurately enter patient billing data into system.
    • Communicate scheduling changes between office and field staff accurately and timely.
    • Ensure maximum third party reimbursement through direct oversight of insurance verification and authorization process.

    Qualifications/Skillsets

    • 3-5 years’ transferrable experience; home health care operations/medical patient admitting experience preferred.
    • Bachelor’s degree preferred.
    • Current, active license to practice as an LPN in this state preferred.
    • Must have a valid driver’s license and current automobile insurance
    • Good communication, negotiation and public relations skills.
    • Autonomous, organized, assertive, flexible and cooperative.
    • Basic computer skills and knowledge of office technology.
